Im a little confused with GPL and Commercial extensions. When the license is GPL and a subscription fee is required, does that mean the extension is free but you need to pay a subscription fee to access it from them? Or you are actually purchasing the extension?

Just looking for a little clarification.

Generally, you are paying for the extension but if the fee is referred to as a subscription/club membership fee you would also typically have access to updates/upgrades/support, etc. for a period of time.

The main difference between the GPL license and a proprietary license, is that under the GPL, you have access to the source code and can modify for your own needs, add features, enhance the product, etc. If you do so and redistribute, you must include the original copyright.
