checkboxes missing from lists- which php option to resolve?

Hi,

If I use a php version above 5.2, I get missing checkboxes [1] on anything that involves a list, as pictured in the linked to screenshot below.

I can solve this by downgrading to php v. 5.2, but then I can't install/use the free AWS S3 storage plugin, since it requires php v.5.3 or above to work.

I'd like to be able to install php.5.6, which is the newest available through my hosting provider. Their support told me that I need to find which module/option I need to enable to resolve the missing checkbox issue. I have these options available to enable [2].

I'm asking here so I might be able to avoid having to test each of these modules/options individually through trial & error, as that would take a long time.

Please advise.
