Does this sound like I've been hacked?....or is it just a problem with my servers?

I get a 404 error on the homepage:

Forbidden
You don't have permission to access /index.php on this server.

Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request

Do you have FTP access to your site? If so, look for your index.php file and make sure it's there.

This could be an issue with .htaccess or something else.

Let me know what you find.
