Download extension with email required

hi to everybody!
i need an extension (component?) for the download of file from my website.
but i need an extension that show some rule for the use of the file that the visitor should download, and that at the end of the rules ask to confirm the rules and to insert the email, so that i can know the email of who download the file.
do you know what can i use?

so i decide to add a stand alone php page withe the code i need
