Recommend a good form builder extension

Please I want to create forms (up to 80 field and checkboxes) on my website and I want the results to be sent to my info@ email once the site visitor submits the form. Please can anyone recommend a very good extension to change this?

RSforms is also a good one. For a simple to use one try Fox Contact.
