Looking for a Paypal or send a check extension

I have a client looking to set-up a form for the user to set-up a payment for a conference. They want the user to select Paypal or Pay By Check. I have the Paypal option working thanks to RS Form Pro, but the Pay By Check I can't get to work. They are on Joomla 1.5 and don't want to upgrade even though RS Form Pro has a payment option, but it is for 2.5 and above. They want the user to be able to print the invoice and send a check if this option is selected. What is the best way to achieve this?
