File permission issues under IIS

OK I am having issues with permissions again under windows IIS7.

I have Community builder installed which allows users to upload a profile image. The system uploads the image and creates a thumbnail and a profile image which are resized from the original file and they are placed in the appropriate directory. Problem is the Thumbnail has the correct permissions and displays fine on the page but the Profile image permissions are not correct and the page displays a red X. if I change the permissions manually in the folder then all is well but I cannot keep doing that every time someone updates or changes their image.
Why is IIS behaving like this? What am I missing that will fix this?

Are you using the FTP layer? What are the permissions before and after you manually fix?

No FTP Layer. The Thumbnail ends up with a permission of READ&EXECUTE and the Profile picture ends up with SPECIAL which, when drilled down, is MODIFY. The folder permissions are READ&EXECUTE and all subitems are to inherit that permission (as far as I know). I am a bit new to this sort of thing. I just cannot understand why the permissions would be different.

check your global configuration for CHMOD preferences on new folders / files

IIS might be more of a pain than its worth, why not move to a unix host?

I am not good with Unix or Linux and this is currently running on my machine at home. At this point it is my best option.
