Click Link to Enter email address and download PDF

Hello All,
I have done some searching through the extension list and searched this forum as well and I have not been able to find a suitable solution.
I am looking for an extension (ideally), that when you click on the link it pops up a quick name/email form and then downloads a pdf.

I.e. "Click here to download the latest free whitepaper"

Therefore, obtaining the interested parties name/email to follow-up with afterwards and providing something free in return.

I would imagine many of the forms components would support collecting the information and redirecting to a page where you can download the PDF. Have you asked on any of the forms component forums?

Yes, that is exactly what I want to have.
I have not found an easy plug-in/module solution yet; hopefully someone can provide their expertise.
