mosforme php plugin error

I have a php file named 'mosforme.php' on my server, and I try calling it on an article on my site typed as '{mosforme 1}' and '{mosforme 2}'. The plugin doesn't work, unless I backspace the 1 or 2 after the mosforme line, and re-enter it, and then the desired plugin will display. But when I save that article to exit, the form and plugin no longer work. It does this repeatedly when the exact same results. Not sure what the problem is.. Thanks in advance :P
