Hello I would like to have a feature on the site which has a Continuing Professional Development (CPD) section which users can input (see screen shot). Is there a component/module which I can tweak which would allow me to do a similar thing or do I need to get someone looking at this from scratch.

I wont users to input into a number of fields, and for in to create a record which it displays. I 3wont the user to be able to create multiple records (these could be views on the same or another screen), and to give each record a number (hours). I then want it to calculate how many hours per year (drop down box), the user accrued.

I know this is quite complicated but I really appreciate any help, any one can offer. I'm even willing to have a go at making it myself, although my PHP skills are rather lacking.
