Ever since I swapped over to a dedicated server Google has indexed my website in a weird way. For instance, if I type into Google "driving from a wheelchair" this is the URL that is now being indexed "http://tetraplegicliving.pennwooding.com/disabled-driving-disability-info", whereas it should "http://tetraplegicliving/disabled-driving-disability-info. I'm not finding this is happening with every single query I make, just a lot of the pages are coming off like this on Google. My primary domain is PennWooding.com service obviously got something to do with this, but it's only happening with this particular website, and I do have quite a few on my server.

Just wondered if anyone knows why it has suddenly started doing this

Some web hosts do strange things if you have more than one domain in your account. When I was using Blue Host (same as Host Gator and a bunch of other web host brands), our first domain was considered the primary website. And if I added any additional domains to the hosting account, it automatically created a sub-domain that was hanging off of the primary domain. So for example, if my primary domain was www.example.com and the 2nd domain I added to the account was www.seconddomain.com, then Blue Host made the second domain accessible at BOTH seconddomain.example.com and www.seconddomain.com.

This appears to be the case in your situation. Google is indexing both your primary domain and sub-domain, which creates duplicate content issues and lots of other strange problems. You can probably solve it with a few htaccess redirect rules.
