Looking for module

Hi all,

I am working on a Joomla 2.5.8 site and looking for module - extension, once filled out and submitted will automatically send a pdf E-book to the user and the filled out form data to the site owners.
Does anyone know of any extensions that are able to do this. Preferably free if possible.
