Go to the Plug-in Manager, open the plug-in "System - Language Filter" for edit, in the tab/slide Basic Options, for the option Remove URL Language Code choose the value No.

What version of Joomla! 2.5.x are you using?

Is this a fresh install or an upgrade? If an upgrade, please provide details.

Also, delete all caches.

I believe it was a 2.5.3 install, then an upgrade to 2.5.4, also no caching in used.

Just a thought... What is the value selected for the option "Language Selection for new Visitors"?

Issue is solved, somehow I overlooked the setting "Remove URL Language Code", set this to Yes and it removed the default language code. Thank you both for the help.
