Username and password field missing

At the administration login page username and password field missing. Any ideas how to bring it back? Thanks.

Solved: Page was hacked and folder administrator/modules was missing

Hacks may have left one or more back doors behind. Did you check your site following the instructions in this sticky post: http://forum.joomla.org/viewtopic.php?f=621&t=582854
