I'm out of ideas so here's my situation:
Multilingual site
english and portuguese (british and using brazilian as a placeholder for now)
Same menus exist for both
Menus are associated
The language switcher works
The language filter is set like this:
Language Selection for new Visitors - Site Language
Automatic Language Change - No
Item associations - Yes
Remove URL Language Code - No
Cookie Lifetime - Session
Add alternate meta tags -No
everything works fine within the english version
whenever I try to switch to portuguese, no matter the page, I get a 404 error
Not sure if it's relevant but the language switcher goes from horizontal to vertical display for some reason
Any clues?

small detail I hadn't noticed, even when I select to Remove URL Language Code the portuguese links still appear with pt in there.
All pages work fine if I have the whole language thing off btw

First, check the multilanguage admin module to see if all is OK.

Attach here a screen capture of the result pop up of the module
Second, make sure you have a template style assigned to each language with the correct mainmenu containing the tagged Home.

Appologies for the late reply, only saw the post when I got home on friday and the new version of the site is still in the localhost of the office pc.
The multilanguage admin module showed me that I had the content language wrong (br instead of BR, didn't know it was case sensitive).
Fixed that, here's the screenshot of the current state:

Set two templates to be the default for each language, didn't know you needed separate ones, and assigned them to the respective menus.
So, status report:
The language switcher isn't working, when in dropdown mode only english shows up there, when in flag mode neither appears.
If I manually type in the would be links for the pt section, 404 error
Any sugestions?

When you changed the tag from pt-br to pt-BR, which was the correct thing to do, it meant you also had to review ALL items tagged formerly to pt-br and re-tag them to the Brazilian Portuguese language. They will now get the correct language tag.

I just wanna thanks both of you. I had the same problem with a dual language site (Italian-German).
I was getting mad !!! But finally this post solved everything. It was just a missing letter in the definition of the German language.
