Looking for Quick Contact Us - Ask a Question extension

Anyone know of an extension - or an easy way to add a side bar box on our site that would look something like this:

Ask a Quick Question

Name:
Email:
Question:

Submit


Want to add to some of our product pages to get visitors to interact. Not looking for an annoying pop up or slider - just a simple box.
