Uninstall Fails on JFolder

I've got a module which fails to uninstall. There is no name, version, date, author, or folder for this module. When I try to uninstall this module, I get "Warning JFolder: :delete: Path is not a folder. Path: {ROOT]/modules/mod_default
Module Uninstall: Manifest file invalid or not found."
How can remove / uninstall / delete this module?

If the folder modules/mod_default does not exist, use phpMyAdmin or a similar MySQL client utility from the cPanel provided by your host. Edit the database table _extensions, find the row where the column 'name' is 'mod_default' or blank and delete it.
