How to use js script with module pricing?

I have 4 pricing plans on the website with 4 'Request it' buttons. How it has to work: if the user clicks on any 'Request it' button, the name of chosen pricing plan should put in contact form. http://joxi.ru/xAevjzKHLWggry

I have js script which can do this, but I don't know how to implement it. Can any one help me and explain how to do this step by step?
