Customer confirmed it was the first time he visited my site today, he uses IE11 and Chrome on S6

Did you asked him to clear cache browser? And also you confirmed that it works.

Problem still persist. It happened to me this time. I have cleared cache - same issue. Removed individual cookies - same issue. Cleared recent history - problem sorted.

Why is this happening and how can this be sorted for good ?

1) First upgrade to 3.4.8
2) Logout from Joomla Administrator.
3) Clear your browser cache;
4) Login to Joomla Administrator.

See if everything is resolved.

Hi
I am on 3.4.8 and facing this issue "The most recent request was denied because it contained an invalid security token. Please refresh the page and try again." on article update. I have done all steps mentioned above. Cleared browser cache and _session table. Repaired _session table anyway. Cleared history. logout and login with admin. Created new admin user and tried using that. tried from a new laptop and a new browser. Even rebooted the server. My server's PHP version is 5.5.31. We had the same problem in Joomla 3.4.7. We waited for 3.4.8 but still have the same issue. Can anyone help?

Try this: In global configuration set Cache time to "15" and clear your complete Joomla data and your browser. Close your browser and restart the one you are using and reload your Joomla site
