On this moment, my Joomla 1.0.15 installation runs on MySQL 4.0.24, which is soon not supported anymore. For this reason, my host is migrating every website to another MySQL server, which runs MySQL 5. Will my website stop working and which actions do i need to take to let my website runs like normal.

The website should run on MySQL 5. You can test the site locally before making the change in live site

Yeah, but the problem is that its been installed on a different database and i need to migratie it, before my host forces me to migrate and my site goes down. Do i need to make changes in Joomla, so that the script knows to look at a different database?

Yes you need to change something, but what depends on your host.

Use Joompack to backup and then restore it to a new database.

The server now is sql-1.*****.nl and will become sql-2.*****.nl, so the hostname changes and the version changes. Nothing more. This can be fixed easely without backuping with a third-party extensions, is it not?

FYI, you've been told the safe way to perform the upgrade, backup and restore.

Anything else is between you and your ISP. Since no one controls your ISP we can't promise you they will not have a massive failure/problem when they "upgrade" you.
