Duplicate entry '228-0' for key 'PRIMARY' SQL=INSERT INTO `j16_modules_menu` (`moduleid`, `menuid`) VALUES (LAST_INSERT_ID(), 0);
SQL =
INSERT INTO `#__modules_menu` (`moduleid`, `menuid`) VALUES
(LAST_INSERT_ID(), 0);
Files Update: SQL error file DB function failed with error number 1062
Duplicate entry '228-0' for key 'PRIMARY' SQL=INSERT INTO `j16_modules_menu` (`moduleid`, `menuid`) VALUES (LAST_INSERT_ID(), 0);

First one first: Simple question: Are you trying to install a module or Joomla?
Th message means that the module tabels are already in the database and that you have to delete these first from the table "menus" with phpmyadmin before trying to reinstall

I had a similar error with the 'extensions' table so I simply checked the record in the SQL table and the only change I had to make was changing the last copyright year form '2012' to '2013'. You could do the same thing with the 'j16_modules_menu' table by just manually checking the values to ensure they match with the ones in the error message.

In my case it was from '2.5.9' to '2.5.10'.
