What is missing from Joomla! is more complete control of parameters that most SEO firms are looking for. 

These items include:
*  Ability to define custom title tags for the browser title for each page within the site
*  Ability to define custom meta keywords and descriptions for each page
*  Alt image
*  HREF title tags generally not defined

For my sites I generally had to hack most of this stuff in so we can have these capabilities (thank god for open source!).  I think what I am looking for is more control over these functions.

Anyone else can suggest stuff that is missing from my list?

In upcoming Joomla! 1.1 (was to be Mambo 4.5.3) these are already done:
-  Ability to define custom meta keywords and descriptions for each page
-  Alt image

I don't see "custom" page titles, still uses content title.
You do have more control over how the title displays:
Site - Title
Title - Site
Title only
This is selectable in the site configuration.

No sure about the last one on your list.

Hey I saw people had provide a solution for enabling SEO and SEF in IIS without the need for a external rewrite filter ...... Any chance this will be included as an optional feature in Joomla. It would be helpful for the non - Techinal users .....
