Anyone able to get sh404sef working with SMF ?

I'm using Joomla, SMF, Community Builder, and the Joomlahacks Joomla-SMF Bridge.

I'm trying to get search engine friendly URLs working in SMF using sh404sef.

The test version from here...

http://download.siliana.net/category.php?id=2

...has support for SMF and they demonstrated it working on this site...

http://dev.anything-digital.com/Forum/

However, I cannot get it working properly.  The menu links all end up pointing to the same URL.

Has anyone gotten it working properly?
