Need to know Form Module or Component Name ?

Is there anyone can help me to find a Form Module where i can create a Registration form where i can put some text fields,Checkboxes,Radio Buttons,List Box,Submit Button and After Registration i want a mail to Registred User and for Administrator and module should be Non-Commercial. I got many but all are creating some problems. Is there anyone can tell me any Form Module or Component to get rid or this problem.
