Need help and advice creating an insurance site

I am new to Joomla and have a few questions.
I'm creating a website based on insurance coverage. We are going to include health plan packages for high school student for when they go to college. Does Joomla have any type of features that will let a customer fill out an online form? In this form they need to fill out:

Last 4 digits of Social Security
First and Last Name
City, State, Zip
PHone no. Email
Premium cost amount encloded
Credit Card Number (visa or mastercard)
Name on account, Signature, Date
Cardholder's complete mailing address

What are the necessary tools that I need?
I appreciate for anyones feedback to get me in the right direction.

You could use Joomla's standard contact form with some additional fields, but you would probably find it easier to use an extension. Contact forms have their own category in the JED: ... tact-forms

A few suggestions: Fox Contact is a simple and quick to implement form that can be used both as a component and a module. Moving up from that, with more power and complexity, have a look at RSForm!Pro. If you need to add workflow and/or custom processing of the captured data, try Fabrik, it has the steepest learning curve, but can be used to develop almost any structured data application in Joomla.
