J DContact Form - Removing Fields?

I'm using J DContact, but I can't find in the php files where to remove fields (specifically the telephone field). It doesn't seem to give any options in the module editor and I'm not sure if anybody here is familiar with it here but any help would be great. I've been ctrl+f values all night.

Looks like you never got a reply. I have just asked the same question. The code is under modules/mod_jdcontact/

you can change the title under the override language options.

it an interesting little module.
