Successful registration not in User Manager

I have had this happen on a client's site twice now. A user has a successful registration and gets an activation email but can't activate/login because the user isn't added to the database. I go to User Manager and there is no record of the registration even though I get a "New submission from 'my-website.com User Registration'!" email. I didn't find anything similar to this on the forums and I was wondering if anyone has had this issue and might know what is causing it or if there is anything I can do/view to get an idea why this is happening.
