Forgot password / user id missing

Hello All!
I do not know what I did, but the login module is missing the "Forgot Password / User id.

I have a screen shot of what is missing. If anyone can suggest how to fix outside of dumping the whole thing and rebuilding that would be great.

From the image you have posted, it looks like you are not using the template Protostar.

The template you are using may be having an override file for the login module. Check whether there is a file named default.php in the directory \templates\your-template-name\html\mod_login. Check the code of that file with the file default.php located in the directory \modules\mod_login\view\default.php.

Now, I have a related question. I had to rebuild my site and manually enter each username. I used the same password for all accounts so they could log in and change but,
I'd like to force each one to change password when the log in the first time.
I cannot find a way to do this.
Do you know where I can find the switch for this?
I am using the latest version :

ADDED info:

I saw an article that states if I change the value or jos_users.requireReset from "0" to "1" it will force a changeing of the password on next login.
I did that, changed the password, logged in. Then looked at the value of jos_users.requireReset. It was still set to "1" but I can now log in with being forced to change password. I would have thought that the value would had reset to "0".
If this flag only works once, then how would I force an all user password change a second time?
