Category and Alias confusion in Joomla 3.02

Good evening, this is my second installation of Joomla 3 - the first time I was able to create URLS like mydomain.com/an-article.html but now in my 2nd installation (the difference being I used installatron rather than doing it manually) I get the category in the URL as well, e.g. http://www.motorhomebusiness.com/2-hire ... -hire.html
I'm really not a fan of having the category in there and really I would prefer it to be just http://www.motorhomebusiness.com/motorhome-hire.html without the '2' in there. Any ideas?
...at the moment I only have one article in there, so I can't believe I have 'used up' my aliases.

Generic Testing using different versions of Joomla 3

I just tested this using Joomla 3.0.0 and I was able to create articles in which the category did not show in the URL
http://domain.com/article

I also just installed a fresh copy of Joomla 3.0.2 and was again creating pages in which the URL did not include the category, as in:
http://domain.com/article

I can get the category alias to show in the URL, but I'm not 100% why it is working

An example of when the category aliases show in the URL

When the above Menu structure is created, the URLs look like this:
* http://domain.com/about/blog-articles/blog-articles-2/3-welcome-to-your-blog
* http://domain.com/about/blog-articles/blog-articles-2/4-about-your-home-page

An example of when the category aliases don't show in the URL

In this example, I don't see the category alias in the url, they simply look like this:

* http://domain.com/3-welcome-to-your-blog
* http://domain.com/4-about-your-home-page

I've dug into this for a bit of time, but I'm not able to give a solid answer on how to ensure category aliases are in the url, or how to take them out.

Does anyone else have input?

I am seeing this on some articles but not others... trying to figure out why.

Edit: I have figured out why

when I navigate to an article using a Link URL (e.g. from a button, or simple href = ) ... using a Link URL like this ...
index.php?option=com_content&view=article&id=18&catid=11&Itemid=183
... i get the strange looking address in the address bar.

but, when I link to an article directly from a menu, I get the nice looking address in the address bar. That is, I get the address which matches my alias perfectly
