SEF Url: 404 - reoccurs frequently after weeks- after NO change

I have a very strange one here. Frequently my friendly urls Urls stop working with 404, after weeks or so.
I could not recognise any pattern. And here it gets even stranger. I just need to reupload my htaccess file.
There is no change necessary in the parms, I just enter a blank line or a comment, so the file gets anewer date. I'm on 3.10.0. Nothing else solves the problem(cache delete or so) an I have this for over a year now. Anyone else? Here is my htaccess, but as said I don't need to change any lines before the upload, just ensure that it has a newer date.

Hi

Sound more like something you should take up to your hosting company. The .htaccess content is not related to Joomla really, just to your web server.
