Protostar JForm Rendering Problems in Frontend

I am attaching a very simple module that demonstrates rendering issues in the Protostar template for some basic JForm field types.

Specifically multiple select lists are forced to have the height of a single row and radio buttons do not work at all.

The select list is a CSS problem and easily resolved - I'm afraid I don't know enough about bootstrap to fix the radio buttons problem.
