They must select no more than one option to submit the form.

I don't want to use a plugin, I want to edit the contact page itself, any idea how to add the above to the contact page? Thanks

Make a contact for each topic. In the Single Contact menu item, set the list of contact to show. A drop-down where the user can select the contact will show.
