If a backup of the Joomla folder is restored into a workstation and a clean copy of the same version of Joomla is expanded into another folder, a utility like WinMerge would allow the two folder structures to be compared and any altered and new files identified.

On the other hand, there are also software audit services like Phil Taylor's https://mysites.guru, previous myJoomla.com, where the first audit is free. Phil also fixes hacked sites for a set fee.
