FPA not recognizing Database

For whatever strange reason when I try to put the fpa up on my site it doesn't recognize the database and therefore cannot run the fpa properly, but it does run. When I checked in with my service provider they were able to put it online and it ran properly. Just doesn't run properly when I do.

The only clue I have is when I checked the error log I saw these below. Wondering if anyone has any idea what might be happening.

Line 4566 is empty in the original fpa-en.php
Have you modified the file, or is something on your system truncating the file?

The error messages points to the file fpa_en.php (underscore)
Have you renamed the file?

Hi

Yes I did rename the file one time. But all of the rest of the instances of the same error was set to fpa-en.php Sorry about that.

And no I am not doing anything to the file. I assume something in my system must be. But I don't understand what.
