Best way to add alt tags to menu link images

I want to add 'rel="nofollow"' to the menu item and also add the alt tag. I have amended the Mod_menu in overrides to change the 'Link Title Attribute' but the issue I have is as I am using a 3rd party menu extension it rewrites it and converts the " as code. What is the correct way to sort this please.

For the nofollow you should just use the option on the menu item
