contact form override

I want to override my joomla contact form to get as below. Where should i effect the changes

The main file to modify would be default.php that is located in the directory \components\com_contact\views\contact\tmpl. Other files that may need modification would be default_address.php located in the same directory.

It is not recommended to modify the files directly as it may be overwritten by future releases of Joomla! Instead of modifying the original files, follow the HTML output override technique as documented at: Note:
I believe that some css changes may be needed. If this is the case, check how the user/custom css can be used instead of modifying the template's main css file.
